Take your time, if your form is dated then you will have up to six weeks in which to complete and return the form. Read through the form first so you have an idea of what’s needed. Don’t fill in the form by yourself. Parent Carers often find it hard to distinguish between their role as parents and the extra care and supervision they and others are actually providing. Get help or advice from a welfare benefits advisor. This will increase the likelihood of your child being awarded the right level of DLA. Also involve friends and families as they can also help you to see the additional needs your child has. Keep a diary for a week before tackling the form, with details of the amounts and types of care your child needs during the day and night. This will be useful when you fill in the form and can be used as evidence. It will also refresh your memory of the care you’re providing. Give as much information as possible. You can write outside of the boxes and attach extra sheets if you run out of space. Don’t assume that if your child has a diagnosis that DLA will know how your child behaves and what additional needs they have. Every child is different. Evidence – gather as much supporting information as possible e.g. letters, reports etc. However do not delay sending the DLA form because you are waiting for a report. Make sure you inform people whose name you are putting on the DLA form for evidence. Also let them know how the condition affects your child. This includes schools, as often DLA will contact them as well. Don’t play down your child’s condition. Include the bad days as these give insight into the full extent of your child’s needs. Say how often your child needs help, rather than how often they get help. Show how your child’s needs differ from those of other children of the same age. Photocopy the DLA form once it is filled in, as well as any supporting evidence you have accumulated. This can help you apply again in the future and/or if you have to appeal the DLA decision.
