Have Your Say: Support from your GP surgery

This is a pilot survey for patients from Bradford-on-Avon & Melksham Health Partnership, Ramsbury and Wanborough surgery and Whiteparish surgery.

Carer Support Wiltshire has been working with GP Practices to improve services for carers. It would really help us if you could provide feedback on your experiences at your surgery.

We are asking you to provide the name of your surgery, but the questionnaire itself is completely anonymous.

The deadline for completing the survey is 15th May 2015. Thank you very much for your support with this work.
